Mendax Forensics – Merchant Policies

1. Overview

Mendax Forensics (“we,” “our,” or “the Company”) provides professional polygraph examination services, credibility assessments, consulting, and training. These Merchant Policies outline the terms and conditions governing purchases, scheduling, and service delivery through MendaxForensics.com.

By booking or purchasing services, clients acknowledge and agree to abide by the following policies.

2. Payment Policy

• Accepted Payment Methods: We accept Visa, MasterCard, PayPal, CashApp, Venmo and approved government or corporate purchase orders.
• Payment Terms: All payments must be made in full prior to the examination unless otherwise stated in a written service agreement.
• Currency: All prices are listed in U.S. Dollars (USD).
• Taxes and Fees: Applicable taxes or processing fees may apply based on jurisdiction or payment method.

3. Scheduling and Appointment Policy

• Booking Confirmation: Appointments are confirmed only after payment of ½ of the full amount due for the examination or receipt of a signed agreement.
• Rescheduling: Clients may reschedule up to 48 hours before the scheduled appointment without penalty.
• Late Arrivals: Arriving more than 15 minutes late may result in forfeiture of the session and will require rebooking at the full rate.
• No-Shows: Failure to appear for a scheduled polygraph session without notice will result in full charge and no refund.

4. Cancellation and Refund Policy

Because polygraph examinations require dedicated examiner time and preparation, refunds are limited:

• Full Refund: Available if cancellation occurs at least 72 hours before the scheduled appointment.
• Partial Refund (50%): Available for cancellations made between 24–72 hours before the appointment.
• No Refund: For cancellations made within 24 hours, no-shows, or completed examinations.
• Exceptions: Refunds may be considered in cases of examiner cancellation, verified emergencies, or duplicate transactions.

To request a refund, contact jennifer@mendaxforensics.com within 7 days of the original appointment date.

5. Service Delivery Policy

• Scope of Services: Mendax Forensics provides polygraph and credibility assessment services for legal, corporate, and private matters.
• Examination Procedures: Each examination follows standardized testing protocols, including pre-test interviews, data collection, and results review.
• Reporting: Written reports are typically delivered within 3–5 business days after completion, unless otherwise agreed.
• Third-Party Requests: Results are released only to authorized parties designated by the client in writing.

6. Polygraph Report Deliverable

The polygraph report will clearly document who was tested and why, including the examinee’s name, the date of the examination, and the purpose of the exam (what the test was meant to address). Any admissions or important statements the examinee made during the process will be noted, since those can provide context for the results. Also included the exact relevant questions that were asked and the examinee’s answers to those relevant questions—recorded as “yes” or “no” only—so the reader can see precisely what was tested. If any question includes a term that could be interpreted more than one way, the agreed upon definition will be clearly defined in the report. If the exam is a statement-verification format, attached will be a copy of the examinee’s written statement that was signed and dated. Finally, included will be the results of the chart analysis (TDA), along with the examiner’s name (and signature if the format allows) and the examiner’s contact information, so the report can be verified or followed up on if needed.

7. Privacy and Confidentiality

We recognize that polygraph services often involve sensitive personal information.

• Confidentiality: All client information, examination data, and results are treated as strictly confidential.
• Data Protection: Reports and recordings are securely stored in accordance with applicable data-protection and evidence-handling standards.
• Disclosure: Information will only be shared with third parties upon written consent or when legally required by court order or statute.
For more information, see our Privacy Policy.

8. Professional Standards and Ethics

Mendax Forensics operates under the professional guidelines of the American Polygraph Association (APA) and applicable state regulations. Our examiners are trained professionals committed to accuracy, fairness, and impartiality. We reserve the right to decline service if client requests conflict with ethical or legal obligations.

9. Liability Disclaimer

Mendax Forensics makes no guarantees regarding outcomes or interpretations of results. Polygraph results are opinions based on physiological data, not absolute determinations of truth or deception. Our total liability shall not exceed the amount paid by the client for the specific service rendered.
